Outreaches patients to schedule appointments. This includes determining what barriers or obstacles exist that prohibit patient from making or keeping appointment and determine what resources may be needed or action that should be taken.
Reviews patient’s charts as needed to determine compliance based on specification or process outlined. Determine next steps based on chart review findings.
Reads, understands and applies HEDIS/HCC specification and/or policy as it relates to compliance and outreach.
Determines possible root cause to issue effecting patient compliance and presents issue and possible solution(s) to management
Conducts work essential to supporting outreach efforts, including but not limited to:
Collecting and verifying compliance data
Recording the results of outreach
Reporting results to management
Educates providers and other staff on quality related activities (e.g. HEDIS, HCC)
Performs other job-related duties as required.
