Perform case building, benefit validation and member telephonic outreach for program enrollment.
Conduct member visits in community locations such as hospital sites to enroll members in care management programs and validate member contact information.
Provide ACD phone management for both incoming and outgoing calls.
Provide coverage, Care Management support and administrative duties for the department.
Schedule member appointments with Care managers in Outlook.
Create, interpret, review, edit and organize departmental audits, reports and job aids.
Identify and communicate relevant documents to the appropriate department(s).
Review documents for accuracy, completeness and timeliness for submission to the appropriate project committees or work teams.
Assists in the development of program and project plans, tracking and management of projects and departmental activities to schedule and cost.
