GRH pharmacists provide clinical pharmaceutical care in a variety of settings: inpatient acute care, complex continuing care, childbirth, rehab, pediatrics, outpatient cancer, outpatient renal and a geriatrics drug information resource program. Pharmacists are decentralized and work on the nursing units as an integrated collaborative team members. Pharmacists attend multi-disciplinary rounds, oversee therapeutic drug monitoring, provide drug information, mentor students and participate in a robust medication reconciliation program. All of our technicians are certified by the Ontario College of Pharmacists and take full responsibility for all of the distribution functions of pharmacy services. GRH has recently implemented the Cerner Electronic Health System which includes Computerized Provider Order Entry and Closed Loop Medication Administration.
